Preparation

 

When you consider tumescent liposuction, you will meet with a dermatologist or plastic surgeon. This saline and epinephrine solution is used as it causes blood vessels to temporarily shrink.

 

This is done as it greatly reduces blood loss during the procedure and reduces bruising, swelling, and pain after the procedure.

 

Once the solution is injected, your surgeon will make a tiny incision in your skin and inserts a cannula which is a small, skinny tube connected to a vacuum-like machine into the fatty layer of tissue. By using back and forth movements, the fat is then sucked through the tube into a sterile collection system. An elastic compression garment is used after the procedure as compression has been shown to helps the skin contract and heal.

 

Recovery

 

The local anesthetic injected into the fatty tissue lasts for about 24 hours following surgery. Local anesthetic is wonderful as it greatly minimizes postoperative pain. When this wears off, most patients usually just need Tylenol to manage their pain.

 

After Tumescent liposuction, the majority of patients are alert and able to function without nausea or grogginess. You will be encouraged to walk immediately after the surgery to improve your healing. You can return to regular exercise three to seven days after the procedure. Provided your work doesn’t include heavy lifting you can likely return to work after a few days.

 

Most of the swelling goes down in a few days. Although you will see improvement within one to two weeks, it takes at least three to four months, and in some cases as long as six months, to see the treatment’s full benefit.

 

Risks

Like all surgery, tumescent liposuction carries both benefits and risks. Complications are rare and typically minor. The most common include skin irregularity, lumpiness, dimpling, loose skin, numbness, infections and scarring. If these do occur, they can be easily corrected in most cases.

 

The frequency of complications is significantly reduced when the procedure is performed by a qualified and well-trained dermatologic or plastic surgeon.

The precursors to liposuction actually date back nearly 100 years, starting in the 1920s when French plastic surgeon, Dr. Charles Dujarier first introduced the concept of body contouring via fat removal. He preformed the first precursor surgery to liposuction in 1926 by removing fat from the knees of a model/dancer.

 

Rather sadly for the patient, Dr. Dujarier removed too much fat and his sutures were too tight. This resulted in gangrene setting in and the patient’s leg having to be amputated. It would take decades of work to start refining the procedure and techniques would improve.

 

In the 1960s, European surgeons tried to cut away portions of unwanted fat in patients but these procedures often resulted in a lot of bleeding and scarring. In order to limit the risks to patients, surgeons would only perform these early fat removal surgeries on parts of the body without too many blood vessels.

 

There were some major changes that occurred in the 1970s. Originally, the procedure was done by removing fat with curettes. This method, known as curettage, was inconsistent. Fast forward to 1974 and the father and son team of Dr. Arpad Fischer and Sr. Giorgio Fischer developed a tool known as the cannula. A cannula is a hollow tube that allows surgeons to bypass blood vessels while suctioning out unwanted fat. Thanks to the Fischers, liposuction as we know it was born although the procedure was initially only performed on thighs.

 

In the late 1970s, French plastic surgeons Dr. Yves-Gerard Illouz and Dr. Pierre Fournier built on the successes and techniques of the Fischers. The surgeons improved the design of the cannula and also developed a highly effective crisscross method for fat removal that improved the overall outcomes for patients. Another innovation of these French surgeons was to apply compression to the body after surgery to support the part of the body operated on. Illous and Fournier were utterly essential for the development of the “wet technique” in liposuction. The wet technique simply means that a saline solution (salt water) is injected into the treatment area prior to liposuction being performed. This makes fat removal easier and helps reduce bleeding.

 

Dr. Fournier also pioneering the use of lidocaine (a local anesthetic) during the wet technique to reduce discomfort during liposuction. All of this set the stage for tumescent liposuction, a procedure fine tuned by California dermatologist Dr. Jeffrey A. Klein. Tumescent liposuction uses saline solution, lidocaine and epinephrine, which helps shrink capillaries. These are injected into the area being worked on as it makes the fat easier to remove, reduces discomfort during treatment, minimized bleeding, and helps prevent excessive bruising.

 

The ‘90s saw new innovations with the use of ultrasonic assisted liposuction, or UAL. UAL was\ developed by Italian professor Michele Zocchi in 1992. This procedure involves the application of ultrasonic energy during liposuction to help remove fat without damaging surrounding nerve tissue. PAL was approved for use in 1998, which involves a cannula that pulses during the procedure to help break up fat. PAL, or power assisted liposuction, features devices that are easier for surgeons to control, allowing for more precise and thorough fat removal. These devices produce rapid, small vibrations to break up fat so that it can easily be suctioned out through the cannula.
